Home from hospital
Accompanied transport home can be provided and the scheme offers practical help and emotional support for up to six weeks following discharge from hospital.
We can make sure you have groceries in the fridge, get prescriptions from the chemist and sit and have a chat if you need someone to talk to.
Referrals come via social services, Milton Keynes Hospital and Bletchley Community Hospital. Clients may also refer themselves. There is no charge for the Home From Hospital service.
